Description of Responsibilities:
The Medical Scribe will take dictation from field staff and transcribe the information into the medical record.
Reporting Relationship

Responsibilities include the following:
- Accurately and thoroughly documents nursing visits and procedures as they are performed by the registered nurse, including but not limited to: Assessments, follow up visits, discharge visits and/or any other visits that may be performed.
- Dictation/faxing/phone calls and clerical tasks. Medical Scribes are asked to prepare letters as directed by the nurse, via dictation or summary of the medical record.
- Medical Scribe also ensures that care coordination communication is created, communicated and properly uploaded into the patient’s chart.
- Medical Scribes may also spot mistakes or inconsistencies in Nursing documentation and will work with the nurse to verify the information in order to reduce errors.
- Medical Scribes ensure that all clinical data, lab requisitions are completed and utilized.
- Medical Scribes must comply with specific standards that apply to the content of Medical records and to the legal and ethical requirements for preparing clinical documents and for keeping patient information confidential.
- Medical Scribes collect, organize and catalog data for nursing quality reporting system and other quality improvement efforts and format for submission. They will assist in developing and maintaining systems to track patient follow up and compliance.
- Attend trainings to quickly assimilate new knowledge into processes and procedures.
- Medical Scribes proofread and edit all the nurse’s clinical documentation for accuracy, spelling, punctuation, and grammar.
- Interfaces with staff in other departments to gather information needed for accurate and complete patient care.
- Performs QA Medical Record audits, as requested.
Minimum Qualifications:
- Effective interpersonal, time management and organizational skills.
- Home Health Experience Preferred.
- Knowledge of medical terminology
- Computer skills that include word processing, and efficient use of the internet and e-mail.
- Must possess excellent oral and written communication skills, with the ability to express technical issues in “layman” terms.
Education and/or Experience:
- Must have a High School diploma or Graduation Equivalent Diploma (G.E.D.) or higher
- Education as a medical assistant (M.A.), preferred
- Proficiency in 10-key preferred.
- Prior experience in a pharmacy or home health company is of benefit.
- Prior experience in a consumer related business is also of benefit.
Job Type: Full-time
Pay: $18.00 - $19.00 per hour
Schedule: 8 hour shift
